Responsibilities: Conduct comprehensive psychosocial assessments for adult day health care participants Develop and implement individualized care plans in collaboration with interdisciplinary team members Provide counseling, crisis intervention, and emotional support to participants and their families Connect clients with community resources, social services, and support programs Monitor and document participant progress, needs, and changes in condition Advocate for participants' rights, dignity, and overall well-being Facilitate family meetings and coordinate discharge or transition planning as needed Requirements: Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program Current Massachusetts social work licensure (LCSW, LICSW, or LSW preferred) Experience working with elderly, disabled, or medically complex adult populations Strong knowledge of community resources, elder care services, and social service systems Excellent communication, documentation, and case management skills Compassionate, patient, and culturally sensitive approach to client care Ability to work collaboratively within a multidisciplinary healthcare team About Us: RespectAdultDayHealthCare LLC is a trusted provider of adult day health services in Winchester, MA, dedicated to enhancing the quality of life for adults in our community.
